Attach a document in FP forms

Good Monday all, we have several forms and want people to add a document to
it how can this be done? We are using FP2003

1. To use the built-in FP function, you site must be hosted and published (via http) to a Windows
IIS web server that has the FP extensions installed and the host allows the function to be used.

2. Any file uploaded, would be to a folder within your web site, it will NOT be attached to an
email.
