I have a site that is setup to use Windows Integrated Auth. If I browse to
the site by IP address it works fine. But, if I browse to it by name I get
prompted to enter the user id/ pwd - and, after few tries fails to
authenticate the user.

What is the cause of this issue and how can I fix it?

Hi,

It seems that Internet Explorer automatically logs you on when you use the
IP address. Please check this KB Article so that Internet Explorer is
properly set up for automatic authentication to happen:

Hi,

Can you please enable Logon failure auditing on the server, and then check
the server's Security event logs to see what is happening?
