Form intermittently asking for userid and password

I publish using FP 2003 onto an apache server with FP extensions enabled. I
have a simple form that intermittently asks for a userid & password when the
user hits the submit button instead of directing them to the confirmation
file. This does not happen on other sites I manage.

Is there something I can do to stop the server from asking when submit is
hit?

What are the server settings for authoring in 2003 for an apache server
running extensions?
